He is a recurring collaborator with John Cassavetes, working with him on The Husbands, The Killing of a Chinese Bookie and Opening Night. As the star of the TV series Run for Your Life, Gazzara has been nominated for three Golden Globe Awards and two Emmy Awards. He won the only Emmy for his role in the TV series Hysterical Blindness.
